ABSTRACT

 

Abducens nerve palsy : when does `vascular` mean `dangerous` ?

Most of the acquired abducens nerve palsies are the result of a supposed MICROvascular event, and usually resolve spontaneously. In other rare situations, infectious, compressive, infiltrative conditions can lead to an abducens palsy. We present a series of patients with sixth nerve palsies related to MACROvascular causes ( caroticocavernous fistula, carotid artery dolichoectasia, intracavernous internal carotid aneurysm, cavernous sinus thrombosis).

The presentation describes the clinical work up, findings and treatment profile and outcome of patients with macrovascular abducens palsy. The clinical indications for urgent imaging in these patients are reviewed, prompting early recognition of these potentially life-threatening conditions.
